Artifact Pottery Order Form

At this point in time, I am only set up to accept orders via telephone and through the U.S. Mail.  However, I can accept Visa and MasterCard.  As my web business grows, I hope to add online shopping.  For now, I am trying to keep my overhead low so I can offer you reasonable pricing.  

So, if you are interested in purchasing some of my work, please do one of the following:  Make a telephone order by calling me at 231-723-3495 (between 9:00 am and 11:00 pm please, Eastern Time Zone) or, print out the following form, fill it out, and follow the directions at the bottom.   If you need assistance with this order form, or have other questions, please call me at the above number, or click here to email me.  Even though my gallery is closed in the winter months, I still run my online business all year.

Shipping, handling and insurance are not included in my prices.  Please add 15% for shipping within the continental U.S. ($7.00 minimum) - other locations, contact me and I will give you a quote on shipping.  I pack very carefully to guard against breakage.  I don't do refunds if your order arrives broken.  If you would like to insure what you are ordering, add an additional $3.00 per $50.00 of value or fraction thereof.

Shipping usually takes a week or two, depending on my schedule and your shipping location.  If you included your email address, I will notify you when your package goes out, and give you the shipper's tracking number, so you can be watching for it.
